n conclusion, the totality of the evidence does not support endorsing CCTA as the noninvasive test of choice in evaluating patients with stable chest pain. Arguably, there is only 1 trial with favorable, albeit not robust, outcome data8; however, the quality and the quantity of evidence is not sufficient to justify the imprimatur of Class I recommendation of the ACC/AHA guidelines. At best, CCTA can be deemed an alternative to standard care (including conventional stress testing) in these patients. Any claim to the contrary is perhaps a case of enthusiasm exceeding the evidence.

Bacterial Endocarditis of Double-Chamber Right Ventricle

A 13-yrs-old girl had fever for last 4 months. She had significant weight loss during this period. Echocardiography revealed she was having double chamber right ventricle (DCRV). There were large number of vegetations attached right ventricular outflow tract (RVOT) and tricuspid valve. A plain computed tomography chest revealed consolidation of lower lobe of the right lung. Blood culture yielded Staphylococcus aureus which was sensitive to ceftazidime and gentamicin. She was treated adequately for infective endocarditis (IE) for four weeks and was sent for cardiac surgery. This is an extremely rare case report of DCRV associated bacterial endocarditis with evidence of vegetations in the RVOT, over the TV and embolization into lung.

DNB is equal to DM ,MD,MS

Medical postgraduate degrees from top non-teaching hospitals like Sir Gangaram, Sitaram Bhartia and others will now be equal to MD/MS degrees from teaching institutions and hospitals like PGI, AIIMS and other state medical colleges.

The Health Ministry on Tuesday approved the long-standing demand of the Diplomate of National Board PG holders for equivalence with MS and MS counterparts.

The new Board of Governors of MCI recently approved this equivalence. Health Minister JP Nadda has now cleared its notification. DNB is a degree awarded by the National Board of Examinations. DNB courses allow postgraduate education outside medical colleges and in non-teaching hospitals

Due to the shortage of medical faculty in medical colleges, the government had earlier decided to equate DNB with other MD/MS, DM/M.Ch qualifications. However MCI’s June 5, 2017 notification on minimum qualifications for teachers in medical institutions regulations stood in the way as it prescribed one-year senior residency and three-year experience as a resident in a medical college for teaching positions.

A ministry source said MCI interpreted rules to say that DNB holders must undergo three years’ junior residency in an MCI-recognised medical college to be able to teach. “MCI refused to count DNB course duration as work experience for teaching jobs,” an official said.

As per the revised criteria, candidates who have undergone DNB training in an institution that runs MCI-recognised postgraduate degree courses or have undergone DNB training in a multispecialty teaching hospital with at least 500 beds, their DNB qualifications will be on a par with MCI-recognised qualifications in a given subject. 
Candidates passing from 100 or more bed hospitals will need to work for one additional year as senior resident in a MCI-recognised or permitted institution for equivalence with MD/MS/DM or M.Ch. In all other cases, candidates will have to work two additional years as senior resident or research associates at CSIR or MCI recognised medical colleges for equivalence.
  • The move will bring relief to Diplomate in National Board (DNB) graduates and boost faculty in medical colleges
  • Of 40,000 medical PG seats in India, 6,500 are DNB. The government plans to increase DNB seats to 22,000 by 2022

Alcohol use is a leading risk factor for disease burden worldwide, accounting for nearly 10% of global deaths among populations aged 15–49 years, and poses dire ramifications for future population health in the absence of policy action today. The widely held view of the health benefits of alcohol needs revising, particularly as improved methods and analyses continue to show how much alcohol use contributes to global death and disability. Our results show that the safest level of drinking is none. This level is in conflict with most health guidelines, which espouse health benefits associated with consuming up to two drinks per day. Alcohol use contributes to health loss from many causes and exacts its toll across the lifespan, particularly among men. Policies that focus on reducing population-level consumption will be most effective in reducing the health loss from alcohol use.

If you zoom a little ,you can see the kingfisher is waiting to catch a fish .I have watched her from dawn to darkness ,it never even once dived into the famous Gandamunda nala !The poor little is blind or there are no fishes in the nala?What a tragedy !We use so much pesticides ,so much soaps ,polythene and toxic exhausts from malls , shops , hotels and industrial exhausts have poisoned the water completely,who will thrive at this level of intense water pollution .You can just forget the survival of tiny fishes which can be swallowed by kingfisher .Even crow,being a clever bird does not drink this water .The crows drink water from small bucket which I keep for them in small garden on my terrace.Currently ,the human race is so busy ,neither understand its protection nor the protection of any other species on earth.What a pity!

An waiting couple

A couple was waiting on the road side for auto rickshaw or bus to go their job .What a good luck! A DTS-423 bus and an auto rickshaw appeared at the same time .The auto made it's speed faster and approached to the couple.As usual ,the male part of the couple stretched his hand to the maximum height ,you imagine as if his fingers were touching the sky and to the out most in the horizon from his body in the hope that the couple is preferring to travel by bus . The bus driver knew but went away because the couple were close to the auto .They had no option left but sat in the auto even without asking the hiring cost .After the auto crawled a few meters,the rickshaw puller asked the male why he was looking at the bus and the auto at the same time .The male told ,'we were wishing to go buy bus' .The puller became angry ,stopped pulling the rickshaw and pulled out the couple physically from rickshaw in place of pulling the rickshaw .The innocent couple stood beside the road .The injustice to the common man passed 70 years after India became Independent .Laws made ,laws broken .None cared and none dared to execute the amendment . Now I am 45+ and still found that the couple is waiving there hands sky high to reach to the comfort .It is a real story.The readers are requested not to ask who were the characters in the script if enjoyed it . .

There happened a competition

It is true and novel.There happened a competition.The conspirators slept happily before night because they could convinced the jury.The jury was bribed so much ,in the dark of night ,neither he/she could see nor could carry all .The Jury also found both his/her below knees were soiled with saliva because of multiple licks. Everything became visible day light .The exhibition ground roared .The competition took place.The true runner-up was awarded. The result shocked the public . The medal was quite large and disproportionate to the body size of runner-up.Along with medal ,a certificate was also given .The claps were from the jury and conspirators .Exhibition ground was locked by security after ensuring that everyone including the winner and the looser were out.Public found the pseudo winner was not able to carry the medal because of it's Himalayan size and was also struggling to carry the certificate at the same time .The murmur from the public whispered to the jury by the conspirators that the elected winner of the day was incapable able to carry the the huge medal and the certificate .The jury ordered the looser to carry the medal to the house of winner .This happened recently and the true story is shared in the cause of public interest.

The two left out

Two people shared the left out inches on either side of a auto driver from Alu Gadi .Auto's small small wheels rolled on the smooth NH-5.The 5yrs-old Bajaj auto crawled like a snake toy for a distance of 4 and 1/2 a kilometers.Red light stopped the auto at Khandagiri Traffic.Both the passengers by the side of auto driver got down.Each extended 10 rupees after a lot of struggle to find change .Green Traffic light signaled. Auto driver only took 10 rupees from one person and requested the both the passengers to equally share the other new 10 rupees by PM Modi .Traffic light went on repeating cycles of red and green ,Both the passengers had no change .They were stuck up on side of the road because they could not decide who would be happy to sacrifice five rupees or who would be happy with extra five rupees along with.I was enjoying this at close distance :The conflict between science(What is practically feasible ) and conscience(what is worth doing as a true human ) was like 4D movie clip .Mobile ring from my house disrupted my attention. I left for lunch .I could not see the verdict .Please answer quickly what you could have done because it was lunch time ,raining slightly and either had no change .
